After this, the lateral pharyngotomy is initiated. The hypoglossal nerve is identified and followed anteriorly to its entrance into the floor of mouth lateral to the hyoglossus muscle. The hypoglossal nerve is freed circumferentially from its surrounding venous structures. The overlying posterior belly of the digastric and stylohyoid muscles is transected. The ansa hypoglossi branch of the hypoglossal nerve is transected. This allows the hypoglossal nerve to be retracted superiorly out of the field.

The anterior border is formed by the pterygomandibular raphe.
